I appreciate that attending a consultation with a doctor with whom you are not familiar can be somewhat daunting and it is my aim to make you feel comfortable and provide you with the opportunity to tell me about the injuries you have suffered and the effects that these injuries have had on your day-to-day life. Below I have provided some information that may be of assistance to you in relation to your medical appointment.
When your Solicitor instructs me to prepare a report on the injuries you have sustained in an accident, I will send you details of an appointment with a clinic location map. You will normally be notified at least 2 weeks in advance of the appointment. Please contact me immediately if you are unable to attend the appointment on 01254-703206. Please note that a charge may apply if you fail to attend an appointment without giving prior notice.
With your appointment letter I will enclose a questionnaire. Please take time to complete the questionnaire as it is used to help me to complete your report. I appreciate that it might be some time since you were involved in the accident and the questionnaire will help to jog your memory before the consultation. Please bring the questionnaire to the appointment. A copy of the questionnaire can be printed from the pdf below.
A map of the clinic location will be sent to you.
It is a requirement that I see a form of identification from you. This would preferably be photo-ID, but if you do not have photo-ID then other forms of ID are acceptable, such as bankcard, official letters to you at your address (e.g. Council Tax bill, utility bill). Please bring some ID to your consultation.
Please arrive in good time for the appointment as this allows the clinic to run smoothly without inconveniencing other patients by late arrival. Clinic reception staff will greet you.
Your consultation will last approximately 30 minutes. We will go through the information on the questionnaire to ensure that I have full details of the accident you have been involved in and the injuries that you have suffered. The consultation will also include an examination of the body areas that were injured. My aim is to make you feel comfortable during the consultation and please feel free to ask questions. Chaperones are available at the clinics if you would like one.
In some cases I am provided with copies of your medical records. When using those medical records only entries that are of direct relevance to any injury sustained at the time of your accident will be included in the report. All other aspects of your medical history will remain confidential. Copy records will be held whilst I prepare the report and the copy records will either be destroyed or returned to your solicitor when your case is complete.
I will use the information that you have provided along with the examination findings to prepare my report on your injuries for use in relation to your claim.
